Since 1972, Crusader Community Health has strived to ensure that all persons in our community have access to quality, primary health care.  Crusader Community Health is a community-based, not-for-profit, Illinois corporation, whose mission is to serve the Rockford area with quality, primary health care for all people in need.

Crusader Community Health physicians are board certified or board eligible in their primary care specialities.  Other Crusader staff are similarly qualified in their professions.

Crusader Community Health is accredited by the Joint Commission on Accreditation of Health Care Organizations.
