This is a professional, direct service position providing team -based, holistic patient care. The Behavioral Health Provider is co-located within the primary care clinic and collaborates as part of the health care team providing treatment to patients with behavioral health needs (mental health, substance abuse, behaviors impacting overall health and functioning) utilizing an integrated care model. Behavioral health services are provided to children, adolescents, and adult patients and their families. The Behavioral Health Provider provides services including assessment, therapy (brief and longer-term), and crisis intervention vis warm hand off while patient is in-clinic and/or at scheduled visits with patients. This position provides direct coordination and collaboration with other behavioral health staff, the primary care provider, and other members of the patients’ care team. This position reports to the Chair of the Department of Behavioral Health.
