POSITION DESCRIPTION

The Family Practitioner is a Doctor of Medicine or Doctor of Osteopathic Medicine who provides primary care services to children and adults as part of the provider team at Crusader Community Health.  Care may be provided to any program or site of Crusader Community Health.  The Family Practitioner will collaborate and consult when necessary with patients, families, Crusader support staff, advanced practice providers, as well as internal and external consultants in order to provide the appropriate high level of care.  This position reports to the Family and Adult Medicine Department Chair and subsequently the Chief Medical Officer.

Care & Treatment

Crusader is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).

This health center receives HHS funding and has Federal PHS deemed status with respect to certain health or health related claims, including medical malpractice claims, for itself and its covered individuals.
