Medical information will be sent, free of charge, to your physician or medical facility. All other requests are subject to fees in accordance with State of Illinois statutes.

Sign up for My Crusader Portal

Sign up for My Crusader Portal at your next appointment. Ask for a username and password at the front desk at your appointment check-in. You must provide a valid email address and phone number to receive text messages.

You can also download the Healow app for access from a mobile device: 

  • Download the Healow app from the Apple Store (Apple users) or Google Play (Android users). 
  • Follow the directions and enter your name.
  • When asked for a practice code, enter  DCEGAA
  • Select the West State Street option, 1200 W. State St., Rockford
  • Follow the directions, which will include a one-time text verification.
  • Set up a security PIN to access your health records.
  • That six-digit PIN becomes your ID and password for the Healow app.


Accessing your child’s account
: Parents/guardians and/or legal representatives may apply for access to their child’s account up until that child turns 12.

At age 12, Crusader will automatically terminate the parent/guardian access. Illinois law requires at the age of 12, a child must grant a parent/guardian access to their health record. 

Visit the front desk to share an updated email address that is different from their parent/guardian/legal representative’s email to access My Crusader Portal. An appointment is not needed.
